Independent Provider Agreement

This Agreement is between The Happy Home Company, LLC and the individual applying to be listed as a Village Helper ("Provider"). The Provider is an independent service provider, not an employee, agent or representative of The Happy Home Company.

The Provider sets their own rates, services, availability and service area; chooses which families to work with; and contracts and is paid directly by each family. The Village does not dispatch work, set mandatory shifts, control rates, or process service payments.

The Provider confirms they are at least 18 years of age, that all information in their application and profile is accurate, and that they will keep their profile current. The Provider consents to identity verification, background screening and reference checks as described in our Background Screening Disclosures, and to renewals we may reasonably require.

The Provider agrees to our Provider Code of Conduct and Safety Guidelines, to maintain any license, certification or insurance required for the services they offer, and to never advertise medical, personal-care or regulated home-care services unless properly licensed.

The Provider is responsible for their own taxes, insurance and legal compliance. We may remove a profile at any time for conduct, safety or compliance reasons. Either party may end this Agreement with written notice.
